Facility Waiver (Adult and Minor)

INSTRUCTIONS FOR GUARDIANS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Minor's information will be entered later. If you have more than 7 minors, you will need to fill out an additional waiver form.

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
